Objava o 90-postotnoj učinkovitost cjepiva protiv koronavirusa američkog farmakološkog diva Pfizera i njemačke tvrtke BioNTech, pobudila je veliku nadu svugdje u svijetu, ali je i ujedno otvorila i mnoga pitanja oko buduće distribucije cjepiva.

Naime, ako cjepivo koristi proteine ili nukleinske kiseline (RNK) koje su na sobnoj temperaturi razgradive, onda ih je što dulje potrebo čuvati u tzv. hladnom lancu jer se na nižim temperaturama njihova aktivna svojstva bolje čuvaju. Analogno je to s, recimo, transportom mesa, samo što temperature u ovom slučaju moraju biti znatno niže.

Naš vrsni znanstvenik i jedan od vodećih virusnih imunologa prof. dr. Stipe Jonjić upozorio je da bi problem mogla biti tehnologija primjene jer se to cjepivo do mjesta upotrebe mora donijeti smrznuto.

Usko grlo

- Taj dio je usko grlo koje će trebati vrlo dobro organizirati i pretpostavka je da zemlje imaju već neku strategiju oko toga. Logistički je problem to što se mora dvaput cijepiti. Logika revakcinacije je dobiti primarnu imunost primitkom prvog cjepiva, a onda se ona drugim cjepivom diže na više razine - objasnio je Jonjić.

Brojni stručnjaci s kojim smo razgovarali ipak uvjeravaju kako Europa, kao ni SAD, ne bi trebala imati problem sa skladištenjem na tako niskim temperaturama, ali da će to biti iznimno veliki problem za siromašne zemlje, Indiju, kao i afričke zemlje.

Iz Pfizera uvjeravaju kako postoji jasno definiran plan transporta i distribucije cjepiva. U Michiganu i u Belgiji nalaze se dva golema pogona, dimenzija nogometnog stadiona, u kojima se nalazi na stotine industrijskih hladnjača iz kojih će se cjepiva dalje distribuirati. Cjepiva će transportirati u specijalnim kutijama u kojima će biti suhi led, odnosno kruti ugljikov oksid, koji je Pfizer također posebno dizajnirao. Svaka ta kutija, koja se može ponovno koristiti, sadržavat će od tisuću do pet tisuća doza.

Zašto se eksperimentalno cjepivo mora skladištiti na temperaturi od minus 80 stupnjeva, dodatno je za "Slobodnu" objasnio doc. dr. sc. Vjekoslav Tomaić, virusni onkolog s Instituta "Ruđer Bošković".

– Općenito bi se cjepiva trebala čuvati na niskim temperaturama kako bi se spriječila njihova inaktivacija, naravno ovisno najviše o tehnologiji koja se koristi. Primjerice, cjepivo protiv gripe se čuva na 4 Celzijeva stupnja – napominje znanstvenik.

Naš sugovornik objašnjava da, kad je riječ o cjepivima baziranim na RNK tehnologiji, kao što je ovo Pfizerovo, ono se mora skladištiti na minus 80 jer na višim temperaturama dolazi do nespecifične razgradnje RNK i cjepivo bi bilo brzo inaktivirano.

– Postoje razlike u strukturi RNK i DNK, što utječe na njihovu stabilnost. RNK je puno kraća molekula od DNK, i uz to jednolančana molekula, za razliku od DNK koja je dvolančana, i zbog toga je DNK više stabilna i općenito je stabilnija na višim temperaturama. RNaze su enzimi koji razgrađuju RNK, a u tijelu se nalaze u slini, mukozi, sluznici, suzama, otpalim stanicama kože, kosi itd., i oni su aktivniji na višim temperaturama, što bi dovelo do razgradnje RNK u cjepivu i ono ne bi bilo više učinkovito – tumači doc. dr. Vjekoslav Tomaić.

Podsjeća i kako postoje studije koje ukazuju da su RNAze aktivne u nekoj mjeri i na minus 20 stupnjeva i mogu dovesti do razgradnje RNK, zato je potrebno cjepivo čuvati na što nižoj temperaturi.

– Osim zamrzivača na -80 °C, ova cjepiva se mogu čuvati i u tekućem dušiku. Što se tiče transporta, bit će potreban suhi led, to je čvrsti oblik ugljikova dioksida, čija je temperatura oko -80 °C, tako da zapravo djeluje kao i zamrzivači ovog tipa na minus 80 – zaključuje virusni onkolog.

Temperatura tekućeg dušika seže do -195 °C. Obično se drži u limenim posudama dizajniranima poput finih bačvi za vino koje se s vremenom nadopunjavaju dušikom. Koriste ga sve naše bolnice i znanstveni instituti.

Hrvatska je, kako je danas potvrdio ravnatelj HZJZ-a  Krunoslav Capak od Pfizera naručila milijun doza cjepiva, a od Astra Zenece 2 milijuna i 700 tisuća doza te 900 tisuća cjepiva od tvrtke Johnson&Johnson

Prof. dr. Dragan Primorac:
"Prema proizvođačima Pfizeru i BioTech njihovo mRNA cjepivo se treba čuvati na temperaturi od -70 do -80 C. Ako je -70 potrebna temperatura čuvanja onda Hrvatska neće imati nikakvih problema, jer suhi led kojeg koristimo desetljećima održava tu temperaturu kako tijekom transporta tako i tijekom čuvanja određenih bioloških materijala. Međutim da je potrebno postići i niže temperaturu, uvijek na raspolaganju imamo tekući dušik koji se drži u posebnim kontejnerima, a čija temperatura doseže i do - 195 C. Tekući dušik sam osobno koristio godinama prilikom smrzavanja stanica i vrlo je jednostavan za korištenje.Važno je znati da se tekući dušik koristi u nizu institucija u Hrvatskoj. Iz svega je očito da čuvanje cjepiva kad jednom dođe u Hrvatsku nikako ne bi smio biti problem - kazao nam je Primorac.

Capak: Spremni smo za dolazak cjepiva

Cjepivo će biti neobavezno i za strateške i ranjive skupine u Hrvatskoj besplatno.

"Trenutak kada ćemo imati cjepivo je sve bliže, mi se ozbiljno pripremamo za to, a sada imamo i nacrt plana. Ne znamo još neke logističke i distribucijske elemente, u kojem ćemo momentu doći koliko cjepiva. Kada budemo to znali onda ćemo taj plan finiširati.Na vrijemo smo se pobrinuli da rezerviramo količine cjepiva za više od 50 posto stanovnika RH. Rang listu nismo još radili, ali sasvim sigurno će prvo biti strateške skupine kao zdravstveni i radnici u socijalnom sektoru. Starije osobe iznad 65. godine, kronični bolesnici i tako dalje.Kada dozname sve detalje po pitanju logistike, onda ćemo definirati sve elemente i završiti plan – rekao je šef HZJZ-a Krunoslav Capak.

Objava o 90-postotnoj učinkovitost cjepiva protiv koronavirusa američkog farmakološkog diva Pfizera i njemačke tvrtke BioNTech, pobudila je veliku nadu svugdje u svijetu, ali je i ujedno otvorila i mnoga pitanja oko buduće distribucije cjepiva.

Naime, ako cjepivo koristi proteine ili nukleinske kiseline (RNK) koje su na sobnoj temperaturi razgradive, onda ih je što dulje potrebo čuvati u tzv. hladnom lancu jer se na nižim temperaturama njihova aktivna svojstva bolje čuvaju. Analogno je to s, recimo, transportom mesa, samo što temperature u ovom slučaju moraju biti znatno niže.

Naš vrsni znanstvenik i jedan od vodećih virusnih imunologa prof. dr. Stipe Jonjić upozorio je da bi problem mogla biti tehnologija primjene jer se to cjepivo do mjesta upotrebe mora donijeti smrznuto.

Usko grlo

- Taj dio je usko grlo koje će trebati vrlo dobro organizirati i pretpostavka je da zemlje imaju već neku strategiju oko toga. Logistički je problem to što se mora dvaput cijepiti. Logika revakcinacije je dobiti primarnu imunost primitkom prvog cjepiva, a onda se ona drugim cjepivom diže na više razine - objasnio je Jonjić.

Brojni stručnjaci s kojim smo razgovarali ipak uvjeravaju kako Europa, kao ni SAD, ne bi trebala imati problem sa skladištenjem na tako niskim temperaturama, ali da će to biti iznimno veliki problem za siromašne zemlje, Indiju, kao i afričke zemlje.

Iz Pfizera uvjeravaju kako postoji jasno definiran plan transporta i distribucije cjepiva. U Michiganu i u Belgiji nalaze se dva golema pogona, dimenzija nogometnog stadiona, u kojima se nalazi na stotine industrijskih hladnjača iz kojih će se cjepiva dalje distribuirati. Cjepiva će transportirati u specijalnim kutijama u kojima će biti suhi led, odnosno kruti ugljikov oksid, koji je Pfizer također posebno dizajnirao. Svaka ta kutija, koja se može ponovno koristiti, sadržavat će od tisuću do pet tisuća doza.

Zašto se eksperimentalno cjepivo mora skladištiti na temperaturi od minus 80 stupnjeva, dodatno je za "Slobodnu" objasnio doc. dr. sc. Vjekoslav Tomaić, virusni onkolog s Instituta "Ruđer Bošković".

– Općenito bi se cjepiva trebala čuvati na niskim temperaturama kako bi se spriječila njihova inaktivacija, naravno ovisno najviše o tehnologiji koja se koristi. Primjerice, cjepivo protiv gripe se čuva na 4 Celzijeva stupnja – napominje znanstvenik.

Naš sugovornik objašnjava da, kad je riječ o cjepivima baziranim na RNK tehnologiji, kao što je ovo Pfizerovo, ono se mora skladištiti na minus 80 jer na višim temperaturama dolazi do nespecifične razgradnje RNK i cjepivo bi bilo brzo inaktivirano.

– Postoje razlike u strukturi RNK i DNK, što utječe na njihovu stabilnost. RNK je puno kraća molekula od DNK, i uz to jednolančana molekula, za razliku od DNK koja je dvolančana, i zbog toga je DNK više stabilna i općenito je stabilnija na višim temperaturama. RNaze su enzimi koji razgrađuju RNK, a u tijelu se nalaze u slini, mukozi, sluznici, suzama, otpalim stanicama kože, kosi itd., i oni su aktivniji na višim temperaturama, što bi dovelo do razgradnje RNK u cjepivu i ono ne bi bilo više učinkovito – tumači doc. dr. Vjekoslav Tomaić.

Podsjeća i kako postoje studije koje ukazuju da su RNAze aktivne u nekoj mjeri i na minus 20 stupnjeva i mogu dovesti do razgradnje RNK, zato je potrebno cjepivo čuvati na što nižoj temperaturi.

– Osim zamrzivača na -80 °C, ova cjepiva se mogu čuvati i u tekućem dušiku. Što se tiče transporta, bit će potreban suhi led, to je čvrsti oblik ugljikova dioksida, čija je temperatura oko -80 °C, tako da zapravo djeluje kao i zamrzivači ovog tipa na minus 80 – zaključuje virusni onkolog.

Temperatura tekućeg dušika seže do -195 °C. Obično se drži u limenim posudama dizajniranima poput finih bačvi za vino koje se s vremenom nadopunjavaju dušikom. Koriste ga sve naše bolnice i znanstveni instituti.

Hrvatska je, kako je danas potvrdio ravnatelj HZJZ-a  Krunoslav Capak od Pfizera naručila milijun doza cjepiva, a od Astra Zenece 2 milijuna i 700 tisuća doza te 900 tisuća cjepiva od tvrtke Johnson&Johnson

Prof. dr. Dragan Primorac:
"Prema proizvođačima Pfizeru i BioTech njihovo mRNA cjepivo se treba čuvati na temperaturi od -70 do -80 C. Ako je -70 potrebna temperatura čuvanja onda Hrvatska neće imati nikakvih problema, jer suhi led kojeg koristimo desetljećima održava tu temperaturu kako tijekom transporta tako i tijekom čuvanja određenih bioloških materijala. Međutim da je potrebno postići i niže temperaturu, uvijek na raspolaganju imamo tekući dušik koji se drži u posebnim kontejnerima, a čija temperatura doseže i do - 195 C. Tekući dušik sam osobno koristio godinama prilikom smrzavanja stanica i vrlo je jednostavan za korištenje.Važno je znati da se tekući dušik koristi u nizu institucija u Hrvatskoj. Iz svega je očito da čuvanje cjepiva kad jednom dođe u Hrvatsku nikako ne bi smio biti problem - kazao nam je Primorac.

Capak: Spremni smo za dolazak cjepiva

Cjepivo će biti neobavezno i za strateške i ranjive skupine u Hrvatskoj besplatno.

"Trenutak kada ćemo imati cjepivo je sve bliže, mi se ozbiljno pripremamo za to, a sada imamo i nacrt plana. Ne znamo još neke logističke i distribucijske elemente, u kojem ćemo momentu doći koliko cjepiva. Kada budemo to znali onda ćemo taj plan finiširati.Na vrijemo smo se pobrinuli da rezerviramo količine cjepiva za više od 50 posto stanovnika RH. Rang listu nismo još radili, ali sasvim sigurno će prvo biti strateške skupine kao zdravstveni i radnici u socijalnom sektoru. Starije osobe iznad 65. godine, kronični bolesnici i tako dalje.Kada dozname sve detalje po pitanju logistike, onda ćemo definirati sve elemente i završiti plan – rekao je šef HZJZ-a Krunoslav Capak.
